How detailed should I make a fever diary for my constant low-grade fever?

I’ve been trying to track down the cause of my constant low-grade fever for weeks now, and my primary care doctor suggested starting a fever diary. I’m a bit unsure how detailed to make it—should I just log my temperature and time, or do I need to include every little symptom I feel alongside it?
