What does optic nerve pallor mean when my visual field tests are still normal?
#1
I’ve been told my optic nerve looks a bit pale during my last two checkups, but my visual field test still comes back normal. My ophthalmologist mentioned the term “optic nerve pallor” and is monitoring it, but I’m confused about how the nerve can show changes without it affecting my peripheral vision yet.

That makes sense you’re seeing. When the optic nerve looks pale it usually means some nerve fibers are thinner or scarred, but the field test checks sensitivity across many points. You can lose a chunk of fibers and still have a normal field early on, especially if the loss is mild or in areas the test maps less well. So pallor can linger even if today’s testing seems normal.

I’ve had something similar and my doctor kept saying optic nerve pallor can reflect past trouble rather than an active crisis. They watched it over months and my vision stayed stable, so there wasn’t a sudden treatment step. The monitoring felt frustrating but it also felt prudent.

One concrete thing I pushed for was OCT imaging to look at the nerve fiber layer. It showed a small amount of thinning in a sector, which lined up with the pale appearance, but the field tests stayed in the normal range. It helped me understand why they’re watching rather than jumping to interventions.

Maybe the problem isn’t the nerve at all, maybe the test is catching something subtle or missing a tiny defect. Either way I keep thinking about how a color change or brightness feels different in dim light, and then the real check is the follow up appointment. Do you ever wonder if this is really the problem or if something else is going on?
