What on-set mistakes have become iconic scenes in film?

I was watching the director's commentary for a film I love, and the director mentioned a specific scene was almost entirely a happy accident from a botched camera move. It got me thinking, how much of what we see as brilliant filmmaking is actually just the crew scrambling to fix a mistake? I'm curious if anyone else has examples where a major technical error or on-set mishap ended up defining a scene for the better.

Ive seen a scene where a stumble in blocking turned into the mood the moment lived in and the director kept it in. It felt real instead of slick and that stuck with me.

One time a camera drifted a bit and we kept rolling and suddenly the extra blur gave the chase a pulse. The editor kept that beat and the scene changed its tone for good.

Another moment was pure luck a sound wobble line up with the cut and the director shrugged and said roll and see. It ended up guiding the cut more than any plan could.
