What signs of ear infection should I watch for in a 3-year-old with night pain?

My three-year-old has been waking up screaming and clutching her ear for the past two nights. Our pediatrician mentioned it could just be fluid from a recent cold, but seeing her in so much pain has us wondering if we should push for a more specific check for an ear infection. I’m not sure what else to look for or if we’re just being overly anxious parents.

My three-year-old had something similar after a cold last winter. The doctor said fluid or pressure can make a kid wake up in pain even if there isn’t a full infection yet. We watched for fever, ear tugging during the day, licking or rubbing the ear, and how long the pain lasted. When she woke at night we followed the pediatrician’s advice for pain relief and tried to keep the room dark and quiet. It settled after a few days, but I kept a note to call the clinic if it kept happening.

We actually asked for an exam because the screams sounded so intense. The nurse looked at her ears but she was thrashing around and hard to read. They said fluid can be there without a clear infection, and sometimes a repeat check is needed. If you’re worried, pushing for a quick ear check or at least a triage call can help. Do you think that’s the right threshold to insist on an exam now?

I’m not totally convinced the pain is an infection either. It could be a short spike of ear pressure from congestion after a cold, or even night-time discomfort that isn’t caused by the ear at all. We kept offering fluids and a gentle bedtime routine and waited to see if it settled. If the pain is real and persistent it tends to show up in other ways too.

The other night I tried dim lights and a white-noise thing to soften the scream and it reminded me how much the sleep environment matters when a kid is uncomfortable. It didn’t fix the underlying issue, but it bought a little time. If this keeps happening, I’d still want a quick check next time the clinic is open, because sometimes the thing we worry about shows up only after a pattern forms.
