How can pelvic tilt cause persistent lower back pain?

I’ve had this persistent ache in my lower back for months now, and my doctor mentioned it might be related to the way my pelvis is tilted. I’m just trying to understand if a slight misalignment there could really be the source of all this stiffness and pain, or if I should be looking into something else entirely.

I had months of lower back ache after years of desk work. I kept hearing posture, but it wasn’t clear what to fix. I tried a short run of core and gentle hip stretches, plus changing chair height. The pain would ease for a day or two and then come back, which was frustrating but at least I could tell what time of day it acted up, and I even rearranged my workspace and started standing more for parts of the day.

I booked a few PT sessions and did a routine focused on hip stability and gentle glute work. It helped a bit, and I started keeping a pain diary to see what moved the needle. Progress was slow and non linear, and some days felt the same no matter what I did.

Honestly I’m not sure it’s the real problem. Some days the ache pops after a long drive or a sudden twist, other days it lingers even when I’m not doing anything different. People talk about pelvis tilt and alignment, but I’ve never felt a clean pattern that would prove that’s the culprit.

Did you get any imaging or a specialist to look at other possibilities? It might help rule out nonmuscle stuff, but I get why you want to know if a tilt could be the culprit.
