How do I adjust saddle height and fore-aft on an endurance bike for knee pain?

I’ve been trying to dial in my riding position on my new endurance bike, but I can’t seem to get the saddle height and fore/aft just right. My knees feel fine on shorter rides, but after an hour I start getting a dull ache in the front of my knee, which makes me think my cleats might be part of the problem.

I feel that. On my endurance bike I kept chasing saddle height and fore/aft and still got a dull ache in the front of the knee after about an hour. I started blaming the cleats, but I wasn’t sure that was it. A few tiny tweaks and rides, nothing dramatic, and I still wasn’t sure what fixed it.

I played with the setback by a couple of millimeters and rode longer, but the knee ache kept moving around. One ride it felt clearer for 20 miles, then it came back, which made me doubt whether the setup was the real issue.

Do you have someone look at a proper bike fit or at least measure your leg lengths and hip alignment? I kept hoping for a quick patch and skipped a real fit, and I wonder if that’s why progress felt slow.

I also drifted once into chasing aero numbers and cadence tests, but the knee pain stayed stubborn. It never felt like one clean fix, more like a mosaic of small things that never lined up for a long ride.
