What am I missing to get the secret ending in New Game+ with all legacy weapons?

I’m trying to get the secret ending in my New Game+ run, but I’m stuck on the final requirement. My understanding is that you need to have all three legacy weapons fully upgraded before the point of no return, but I only have two and the third just won’t drop from the last boss in the sunken archive. Am I missing a hidden trigger, or is my save bugged?

In my NG+ I had two ready, the third wouldn’t drop, and I swore the trigger wouldn’t fire until I hit a specific sequence in the sunken archive. It finally appeared after I thought it might be a bug.

I stuck with two full upgrades and farmed the last boss for hours, but the third never dropped. I kept a mental log of boss fights and loot RNG and eventually gave up on that run and started over.

Do you remember whether you did the optional quest in that wing to adjust loot? I seem to recall something about a hidden trigger tied to it.

I keep thinking maybe the problem isn’t the drop at all but whether you’re actually at the right checkpoint; I drifted away from the map for a while and came back to a different loot pool, but I’m not sure if that’s real.
