What can I do to keep tonal balance and luminosity in black and white portrait?

I’ve been trying to get the look of a classic black and white portrait in my digital work, but my conversions always feel flat and muddy. I’m struggling to understand how to control the tonal relationships between, say, the subject’s skin and a dark background without losing detail or that certain luminous quality.
