What could cause persistent elbow patches that won't smooth with moisturizer?

I’ve been dealing with these persistent, rough patches of skin on my elbows for months now, and my regular moisturizer just isn’t making a dent. It doesn’t itch or look too red, but the texture is really bothersome and it just won’t smooth out. I’m wondering if this is something more than just dry skin that needs a different approach.

I had a patch like that on my elbow last year. I switch to a thick ointment at night and I tried a mild exfoliant every now and then. It slowly felt smoother.

I am starting to think it might be more than dry skin. It stayed rough and did not itch much and I kept hoping moisturizer would fix it but no luck yet. Some people mentioned eczema or keratosis pilaris but I am not sure.

Could this be something like eczema or keratosis pilaris rather than plain dry skin?

One night I forgot to moisturize and slept with a sleeve covering the elbow and when I woke it felt a touch softer I kept doing that for a week but I am not sure if that means anything
