What could clear ear fluid in my 18-month-old mean for a perforated eardrum?

My 18-month-old has been pulling at his ear a lot this week, and yesterday I noticed a bit of clear fluid when he woke up from his nap. He doesn’t have a fever and isn’t acting overly fussy, but I’m worried this could be a sign of a perforated eardrum. I’ve read that sometimes the pressure from an infection can cause a small tear.

I get the worry. My 18 month old did a lot of ear pulling and woke with a clear fluid once too. He had no fever and wasn’t inconsolable, which made it feel off. We used the after hours number and were told to monitor and come in if the discharge stuck around or if he started to drink poorly. The clinician mentioned that sometimes a small tear can happen after an ear infection and pressure, even if there isn’t a lot of pain. We waited about a day; the discharge faded and he ate and slept okay, but I was still anxious.

We did end up at urgent care. They checked his ears and said there was a middle ear infection with fluid behind the drum, no obvious perforation. They gave acetaminophen for pain and advised fluids and rest, with a plan to see his regular pediatrician in a few days. Within two days he seemed calmer and the discharge stopped.

Could it be something else entirely? I keep wondering if the ear pulling is just because teething or a skin irritation behind the ear, and the clear fluid is from a runny nose dripping into the ear via the eustachian tube. It’s hard to know what’s the real problem when the child can still be mostly fine.

Red flags to watch for include fever, dehydration (less wet diapers, not drinking), severe or persistent ear pain, vomiting, or discharge that’s thick, yellow, green, or foul smelling. If any of those pop up, go to urgent care or ER sooner rather than later.
