What counts as a whole grain for heart health?

I’ve been trying to eat more whole foods for my heart health, but I honestly get confused about what actually counts as a whole grain. My oatmeal at breakfast seems right, but then I see “multigrain” bread at the store and I’m not sure if that’s giving me the same benefit.

I used to grab multigrain bread when I’m in a rush, then I learned to flip the bag and read the ingredients. If the first ingredient isn’t a true grain, it’s not giving me what I want.
