What does therapeutic drug monitoring tell me about my antidepressant levels?

I’ve been on the same antidepressant for years, but lately I’ve been wondering if it’s still working like it should. My doctor mentioned the possibility of therapeutic drug monitoring to check the levels, but I’m not sure what that would really tell us or if it’s worth the extra blood work.

Yeah I actually looked into that when my antidepressant stopped helping as much. The idea of a blood test to check levels sounded comforting, but I wasn’t sure what it would change in practice.

A couple of friends did it and the results were normal, and they still had to adjust the dose or switch meds. It felt like a useful check to rule out a rare issue, but it didn’t magically fix things for everyone.

Therapeutic drug monitoring checks trough levels to see if you’re in a known therapeutic range and to catch problems like poor absorption or when you’re not taking it consistently. For many antidepressants though, the link between level and effect isn’t strict, so a “bad” level doesn’t always explain symptoms and a “good” level doesn’t guarantee you’ll feel better.

Lately I’ve been wondering if the struggle isn’t the med alone but sleep, routine, or stress too. If you tried tightening up day-to-day stuff and saw any change, did it change how you felt on the meds at all?
