What rotator cuff issue could cause shoulder pain when reaching a high shelf?

I’ve been dealing with a nagging pain in my shoulder for months now, and my doctor mentioned it might be related to a rotator cuff issue. I’m just not sure what to make of that, since the ache comes and goes but really flares up when I try to reach for something on a high shelf.

I had a nagging shoulder like that too last year. It would hurt most when I reached up to grab something from a cupboard. I iced it, backed off overhead work, and did a few weeks of gentle PT exercises the physiotherapist gave me. It helped a bit, but the flare ups would come back if I skipped the warmups.

I googled symptoms and convinced myself it was a tear, then worried about surgery. In reality, it could be impingement or just tendon irritation from repetitive use. Sometimes people worry it's a rotator cuff tear.

I tried some simple external rotation exercises with a resistance band, one set in the morning and one at night. Pain reduced for about a week, then came back when I forgot to do them. It wasn’t a cure, but it reminded me the shoulder is there.

Do you notice weakness when lifting the arm to the side or reaching across your body? I ask because my doctor kept bringing that up, and it would help if you could tell.

Another time I thought it was just aging, then a little pain after mowing the lawn made me realize it was the shoulder, not my back. The high shelf thing is real. I started paying attention to posture and how I carry bags.

I know this is frustrating. My plan was to get a second opinion with a PT, but I stalled; still dealing with it months later.
