What should I do about my 3-year-old's intense tantrums and head banging?

My 3-year-old has started having these intense, sudden outbursts where he screams and hits his head against the floor or the wall when he doesn't get his way. It’s terrifying to watch, and I don’t know if this is just an extreme version of a normal tantrum or something I should be really concerned about.

I hear you. It’s awful to watch. When my kid did something similar, I focused on safety first—softening the space, guiding their body slowly, and talking in a calm, short voice. After the moment, I wrote down what happened: what had changed that day, how long it lasted, things that seemed to set it off. It helped me spot patterns and feel a little less helpless.

This kind of head banging is scary enough to be taken seriously. It isn’t automatically dangerous if injuries aren’t happening, but it can lead to injuries. I started keeping a simple log of each episode and looked for triggers like hunger, fatigue, noise, or big changes. I’d book a pediatrician appointment soon to rule out medical issues or discuss whether a developmental check is needed. If it happens more often or lasts longer, push for same-day guidance.

Maybe the loud outburst isn’t the real problem. Could sleep, meals, or sensory overload be the bigger issue? I once found that a stricter nap schedule and a quiet, dim routine helped a bit, but then I wondered if I was addressing the signal rather than the cause. Still not sure.

Yes, that sounds terrifying. I would call the pediatrician and ask for guidance now, especially since it’s new and intense. In the meantime, keep him safe—soft surface or padding near hard edges, and avoid letting him head any harder than necessary. If you notice loss of consciousness, confusion, severe swelling, or repeated hits, seek urgent care.
