What should I do if my 4-year-old has ear pain and fever?

My 4-year-old has been complaining that his ear hurts for two days now, and he’s been running a low fever. I’m trying to figure out if this is just a typical ear infection or something I should be more worried about. He’s had them before, but this time he’s also pulling at his ear a lot and seems more irritable than usual.

That combo sounds scary familiar. A few days of ear pain with a fever and a kid who keeps grabbing at the ear is often an ear infection, especially after a cold. It’s not automatically dangerous, but watch for red flags like a fever over 102 for more than a day, refusing fluids, or waking up vomiting. If any of that shows up, call the pediatrician.

From my own layperson experience, I pressed fluids, offered easy foods, and used a fever reducer if the temp was high. If the pain is stubborn or the fever sticks around, that’s when I’d want a check. If he won’t drink or you’re worried about dehydration, that’s a sign to seek care.

We did a same-day telemedicine check after two days of pain. The clinician suggested monitoring and to come in if dehydration showed up or the fever stayed high. I did notice the fever eased a bit after meds, but the ear pain didn’t fully disappear.

Part of me wonders if there’s something else, like a sore throat or just general irritation; the ear tugging is so specific though. Do you think this is the real problem, or could we be missing a bigger issue?
