What to expect when adding a LAMA to my asthma care?

I’ve been diagnosed with moderate persistent asthma, and my doctor is recommending I start on a long-acting muscarinic antagonist as an add-on therapy to my current inhaled corticosteroid. I’m a bit hesitant because my current regimen feels manageable most days, though I do still have breakthrough symptoms a couple times a week. I’m wondering if others have had a similar step-up in their treatment and what the adjustment was like.

Managed to start tiotropium as an add on to my ICS. Took about two to three weeks before I noticed anything, mostly a bit less nighttime coughing. Dry mouth was annoying at first, I had to sip water more often, but it faded after a while. My rescue inhaler use dropped some days, but not every day.

I was hesitant too. The breakthrough symptoms were still there a couple of times a week. After a month I felt the days weren’t much different, but maybe the mornings were a touch smoother. Not sure if that was the med or just luck with weather and pollen.

I went ahead because my doctor said it could help; I got a new inhaler technique reminder and a spacer check. It was clunky at first, more gadgets to manage. After six weeks I didn’t feel a big difference, and I started worrying about cost and long-term side effects more than any benefit.

Could it be that the real issue is triggers like allergies or cold air rather than the asthma itself, and I’m chasing a tweak that won’t fix the root cause?
