Why does my child have breath-holding spells during tantrums?

My 4-year-old has started this new thing where he holds his breath for a few seconds when he’s really upset and then lets out this huge gasp. It’s terrifying to watch, and it happened twice this week after a tantrum over getting dressed. I know it’s supposedly not harmful, but seeing his lips go a little blue for a moment makes my heart stop every time. I’m just wondering if other parents have been through this and what it was like for them.

I’ve been there. Our four year old did the same thing after a meltdown over something tiny like a sock. He held his breath, lips looked a bit blue for a second, and then he let out this loud gasp. It was terrifying to watch. The pediatrician said these episodes are common in kids that age and usually not dangerous, but I still kept a careful eye on him and tried to calmly steady the moment when it happened.

We tried not to trigger the spiral—when we saw signs of a melt‑down coming we tried to pause, talk softly, and redirect with a game or a snack. The first few times he still did it, but over a few weeks it happened less often and felt less dramatic, even if it still scared me every time.

I keep wondering if the real issue is something like sleep or fatigue since it seems to happen after a long day. It’s hard to know, and I don’t want to turn every scream into something medical, but it does make you doubt what’s normal.

If the lips stay blue for more than a moment or if there’s fainting or trouble breathing afterward, I’d call the pediatrician or urgent care. For us, it calmed down as he grew, but it was definitely a roller coaster those first few times.
